Common questions
What is a MAC locality and why does my ZIP matter?
Medicare splits the country into about 110 payment localities, each with its own rates that reflect local practice costs. Two practices in the same state can sit in different localities and be paid differently for identical services. Your ZIP code determines your locality, and this site resolves it for you.
Why do NPs and PAs show 85%?
By federal statute, services billed by nurse practitioners and physician assistants under their own NPI are reimbursed at 85% of the Physician Fee Schedule amount. The 100% columns apply to physicians (MD/DO) and to services billed incident-to under physician supervision where the rules allow.
Why are these numbers useful if I do not take Medicare?
The Medicare fee schedule is the reference point the rest of the market prices against. Commercial insurance contracts are commonly negotiated relative to it, so knowing your local Medicare rate tells you whether a commercial rate is strong or weak for your area.
Where do these rates come from?
Directly from the CMS CY2026 Medicare Physician Fee Schedule carrier files and the official ZIP-to-locality crosswalk. Rates shown are the participating, Medicare-allowed amounts for your locality. Updated when CMS publishes new rates each year.
